Chandan Kundavaram, MD: Dr. Chandan Kundavaram is excited to return to the Valley to provide comprehensive urologic care to the community. He recently completed advanced fellowship training in minimally invasive urologic surgery including endoscopy, laparoscopy, and robotic surgery to treat both benign and malignant urologic conditions at the renowned University of Southern California Institute of Urology and the UCLA Medical Center.

John Mai, MD: Dr. John M. Mai, MD is a urologic surgeon who grew up in Omaha, Nebraska. He attended the University of Nebraska and graduated with a double major in biology and psychology. He then received his medical degree from the University of Nebraska Medical Center in Omaha while on multiple scholarships.

Paul M. Papoff, MD, FRCS(C): Dr. Papoff is an assistant clinical professor at the University of Arizona and has over thirty years of experience in urology. He has a special interest in geriatric urology and urologic cancers. He strives to communicate to all patients about their urologic problems and treatment options.

Amy Schlaifer, MD: Dr. Schlaifer is a urologic surgeon from Prescott, Arizona. She trained at Loma Linda University Medical Center where she was awarded for her research contributions in prostate cancer and female urology. Her training allows her to provide a wide array of surgical skill including female and male reconstruction as well as low dose radiation stone treatment.

Jennifer Klauschie, MD: A graduate of Mayo Clinic Arizona with fellowship certification in Female Pelvic Medicine and Reconstructive Surgery (Urogynecology). Her surgical expertise encompasses all routes including abdominal, vaginal, laparoscopic, hysteroscopic, and robotic procedures.

Loren L. Faaborg, MD, FACOG:A graduate of the University of Nebraska Medical Center, Loren L. Faaborg, MD, FACOG enjoys improving the quality of life for women by providing cutting edge surgical techniques and specializing in general gynecologic needs. He is skilled in advanced laparoscopic techniques, difficult pelvic surgeries, transvaginal reconstruction, and urinary incontinence procedures.

Debbie Sullivan

Tyler W. Chavez, MHS, PA-C: Tyler is a graduate of the Physician Assistant Program at Duke University School of Medicine with a Masters of Health Science and a Physician Assistant Certification. After graduation he continued his training as Surgical Resident gaining experience in all areas within the Department of Surgery at Duke. Tyler also has a Bachelors of Science in Neuroscience from Washington State University in Pullman, WA.

Katie Gillies, MMS, PA-C: Katie is a graduate of Midwestern University in Glendale, Arizona with a Masters of Medical Science in Physician Assistant Studies. Prior to becoming a physician assistant she graduated summa cum laude from the Honors College at Ball State University with a Bachelors of Arts in Spanish and Secondary Education. She also holds a Masters certificate in English as a Second Language (ESL) from Aurora University. She previously spent 6 years teaching high school Spanish and ESL in the Chicago suburbs prior to moving to Arizona and changing careers to pursue medicine.

Debbie also earned a doctorate degree in Adult Education from Capella University in Minneapolis, MN.
